Who Are We?
Agency offers Consumer-Directed Service Facilitation under the MR, EDCD and DD waivers through the Division of Long Term Care Department of Medical Assistance Services. This program enables you to maintain your Medicaid personal care services and hire your own staff to conduct your personal care needs. Help assist you in the guidance of learning more about the program and then maintaining you in the program once you have been approved.

In addition we offer Personal Emergency Response Systems. For times when your loved ones are left alone. We are always there 24/7 in case of a fall.

Accepted payment:Medicaid Comments:Medicaid is billed directly so there should be no cost if you are Medicaid eligible. Private pay is offered for Personal Emergency Response System.

How to Access Services:Call for information or to set up appointment. When calling for services you can dial EXT 1 for our Intake Line. Here our Intake coordinator will address your personal healthcare needs. In addition you can go online to and create an intake via our online services.
