Who We Are
The Senior Transitions program will primarily serve patients 65 and older with emotional and behavioral health issues such as depression, bipolar disorder, anxiety disorders and schizophrenia. Patients as young as 55 years old may also quality for admission if they suffer with chronic mental health issues and or disability. The goal of the program is to stabilize the patient and get them back home or to their original care setting.
