Who We Are

Rockbridge Area Community Services offers Case Management Services for adults with serious behavioral health conditions. Case Managers enhance the quality of life for those recovering through treatment coordination, support and advocacy.  Services include:

  • Assessment of needs,

  • Assistance to develop and work on goals, 

  • Coordination of services including referral and linkage to other services,

  • Monitoring service delivery and progress.

Case Managers make outreach visits, assist with applying for benefits and advocate for the client within the agency and the community.
