Who We Are
Staff at The Community Living Program in Montross, offers Care Coordination. Service Outreach to Seniors (SOS) links seniors and caregivers to the community based resources needed to remain independent. The SOS program provides Care Coordination to assess, educate, make referrals, and provide follow-up to assure access to available services is received. Provides services to individuals and families living in Northern Neck.
